  3.                           OTHER PASSPORT INFORMATION

  6.          Obtaining A Passport in an Emergency
  7.  
  8.            Passport  agencies  will  expedite  issuance  in  cases  of
  9.          genuine, documented emergencies. If  you are leaving within 5
  10.          days and need  a passport, You can pay  to have your passport
  11.          delivered to  you by express  mail. But you  must arrange and
  12.          pay for  this in advance.  Check with the  courthouse or post
  13.          office  that accepts  your  application  or with  the nearest
  14.          passport agency for specific details.

  16.            If  you plan  to travel  abroad frequently  or if  you stay
  17.          overseas  for  long  periods   of  time,  your  relatives  or
  18.          associates in  the U.S. should have  valid passports as well.
  19.          That way, should you become seriously ill or involved in some
  20.          other emergency, they could travel without delay.

  22.          Diplomatic and Official Passports

  24.            If  you  are  being  assigned  abroad  on  U.S.  Government
  25.          business  and are  eligible to   apply by  mail for  a no-fee
  26.          passport  (no-fee  regular,  official,  diplomatic), you must
  27.          submit  the mail-in  application form,  your authorization to
  28.          apply for a no-fee passport,  your previous passport, and two
  29.          photographs  to the  Passport Agency  in Washington  D.C. for
  30.          processing.

  32.          Additional Visa Pages

  34.            If you  require additional visa pages  before your passport
  35.          expires, obtain  them by submitting  your passport to  one of
  36.          the  passport  agencies  listed  in  another  chapter. If you
  37.          travel  frequently  to  countries  requiring  visas,  you may
  38.          request a 48  -page passport at the time  you apply. There is
  39.          no  additional  charge  for  extra  pages  or  for  a 48-page
  40.          passport.

  42.          Change of Name
  43.  
  44.            If you have  changed your name, you will  need to have your
  45.          passport  amended. Fill out  Form DSP-19, Passport  Amendment
  46.          Validation  Application, which  is available  from any office
  47.          that  is authorized  to accept  passport applications. Submit
  48.          the DSP-19  along with proof  of the name  change (a marriage
  49.          certificate, divorce decree, or certified court order) to the
  50.          nearest passport agency. There is no fee for this service.

  52.          An Altered or Mutilated Passport
  53.  
  54.            If  you mutilate  or alter  your U.S.  passport in  any way
  55.          (other than changing the personal notification data), you may
  56.          render  it invalid,  cause yourself  much inconvenience,  and
  57.          expose  yourself  to  possible   prosecution  under  the  law
  58.          (Section 1543 of Title 22 of the U.S. Code).

  60.            Mutilated  or  altered  passports  should  be  turned in to
  61.          passport  agents,   authorized  postal  employees,   or  U.S.
  62.          consular offices abroad.

  64.          Loss or Theft of a U.S. Passport
  65.  
  66.            Safeguard   your  passport.   Its  loss   could  cause  you
  67.          unnecessary  travel  complications  as  well  as  significant
  68.          expense.  If your  passport is  lost or  stolen in the United
  69.          States,  report the  loss  or  theft immediately  to Passport
  70.          Services,   1425  K   Street,  N.W.,   Department  of  State,
  71.          Washington D.C. 20524 or to the nearest passport agency.

  73.            Should your  passport be lost or  stolen abroad, report the
  74.          loss immediately to the local  police and to the nearest U.S.
  75.          embassy or consulate. If you can provide the consular officer
  76.          with  the information  contained  in  your passport,  it will
  77.          facilitate  issuance of  a new  passport. Therefor, photocopy
  78.          the  data page  of your  passport and  keep it  in a separate
  79.          place.  In addition,  leave  the  passport number,  date, and
  80.          place  of issuance  with a  friend or  relative in the United
  81.          States.
